Who Qualifies for Home Care?
Home health care is a Medicare Part A benefit. The Medicare Conditions of Participation require the patient to be homebound, under the care of a physician and have a need for
skilled care (provided by a nurse or therapist) on an intermittent basis.
How Do I Arrange for Home Care?
We accept referrals directly from patients, families, physicians, nurses, case managers, discharge planners, social workers, therapists, and insurance companies.

What Happens After I Call?
A registered nurse or therapist will promptly visit your home to perform a complete assessment that will determine what kind of care is required to meet your initial needs. Your team of healthcare professionals will continually reassess your needs, plan and provide your care, monitor and evaluate your progress, and coordinate follow-up care.
Does Insurance Cover Home Care?
AtlantiCare Home Care is a preferred provider for Medicare and Medicaid in Atlantic and lower Camden counties. We are also a preferred provider for many managed care organizations
and insurance plans that qualify us to provide services throughout southern New Jersey for patients with those plans.
Our case managers will contact your insurance carrier to determine your home health coverage.
