When you contact AtlantiCare HomeCare, we work with your doctor to create a personalized care plan for you. Next, we assemble a team of healthcare professionals to bring the care you need into your home. Our care team consists of:

Skilled Nurses (RNs and LPNs)

• Specially trained in injections, medications, dressings, wound care, catheter care, stoma care, pain management, bladder and bowel training
• Educate and train to promote healthy behavior and involve you and your family in your care

Physical Therapists

• Perform restorative therapy to help you regain your strength and maximize your physical ability
• Develop, evaluate and instruct in a home exercise program
• Train in walking and transfers
• Instruct in use of assistive devices

Speech/Language Pathologist

• Assist with improving speech/language and other communication skills
• Perform therapy for swallowing disorders

Occupational Therapists

• Provide therapy to help you perform the tasks and activities of daily living
• Cognitive skills training
• Fit and train in the use of orthotic and self-help devices

Medical Social Workers

• Connect you with appropriate community resources 
• Provide counseling

Home Health Aides

• Assist with personal care
• Perform the plan of care developed by your healthcare professional
